3. The Wales Inclusion Health Programme for Primary Care webpage Inclusion health can include any population group at the sharp edge of health inequalities. These groups experience stigmatisation, social exclusion, discrimination and experience “severe, overlapping and multiple disadvantages” across their lives, significantly increasing their risk of poor health.
