Deep End Cymru has explored the option of a link worker for Deep End practices.

We know that a significant amount of consultations involved more than just health, encompassing employment, living conditions and financial issues. We would like to see a link worker available to practices that has good local knowledge of support and is able to help the patient address some of problems they are facing in order to improve their health outcomes.
